Please fill out the patient history form and click the submit button on the bottom of the page.

We require the history form to be submitted at least 24 hours prior to the appointment. This allows the doctor to:

• review the patient's information prior to starting the exam
• devote the appointment time to evaluating and explaining the patient's ocular health and visual needs
• stay on schedule
